What to Bring to Your First Appointment 

We will need a photo ID, insurance card, and a list of medications you are currently taking. You will be asked to complete a patient registration form and medical and dental history forms. Please come in 15 minutes early to fill out all necessary paperwork.

Coverage & Insurance 

Dental insurance is confusing. We accept every type of Insurance at Evergreen Dental. We will try to navigate and simplify the insurance benefits that you are entitled to. REMEMBER YOUR BENEFITS EXPIRE ON A YEARLY BASIS so BOOK TODAY to use them to their full potential. You paid for them so use them!

Are there Payment Plans Available?

Dentistry is expensive and we at Evergreen Dental understand that. That is why we have setup payment plans that work with you. We can do payments in installments where we split the cost of the appointment into three equal payments over a three-month period for large procedures over 800 dollars. One-third is paid at your first appointment, one-third will be paid on the completion of treatment and the last third will be paid within a month after the procedure.
